By the late 1980s, hospitals around the world were quietly experiencing a crisis. Healthcare workers — nurses, surgeons, dental hygienists — were developing mysterious allergic reactions. Some had rashes on their hands. Others had severe asthma attacks at work. A few went into anaphylactic shock. The common thread: latex gloves.

This is the story of the latex allergy epidemic — and how it reshaped modern glove manufacturing.

What Is Latex Allergy?

Natural rubber latex contains over 200 proteins. Some of these proteins are allergens — they trigger immune system reactions in susceptible people. There are actually two distinct allergic reactions to latex:

  • Type I (IgE-mediated) allergy — the dangerous one. Causes immediate reactions: hives, asthma, anaphylaxis. Can be triggered by tiny airborne particles.
  • Type IV (cell-mediated) allergy — a delayed contact dermatitis. Less dangerous but more common. Caused by chemicals used in rubber processing (not the latex proteins themselves).

Type I latex allergy is what made the 1990s epidemic so serious.

The 1980s Setup for Disaster

Several trends converged in the 1980s to create the perfect storm:

  1. Universal precautions. In 1985, the U.S. CDC issued guidelines recommending that all healthcare workers wear gloves when handling blood or body fluids — driven by the emerging AIDS crisis. This was a major public health win, but it meant a massive increase in glove use.
  2. Powdered latex gloves were the standard. Powder made gloves easier to put on, but it also aerosolized latex proteins, which workers then breathed in. This dramatically increased sensitization rates.
  3. Increased glove manufacturing to meet the new demand, much of it using high-protein latex formulations.
  4. Children with spina bifida — who had multiple surgeries and medical procedures from birth — were developing severe latex allergies at rates approaching 50%.

By 1992, healthcare worker sensitization rates were climbing to 8-17% in many studies. Some hospitals reported anaphylactic reactions during surgery when patients or staff encountered latex gloves.

The Peak: 1990s Crisis

The mid-1990s saw a flurry of deaths and severe injuries:

  • Multiple intra-operative anaphylaxis deaths in patients with previously unknown latex allergy
  • Healthcare workers developing occupational asthma severe enough to end their careers
  • Children with spina bifida facing life-threatening reactions during routine procedures
  • FDA receiving thousands of adverse event reports

By 1997, latex allergy had become a major occupational health issue, especially in nursing, dentistry, and surgery.

The Response: Multiple Strategies

1. Powder-Free Glove Mandates

The single most effective intervention turned out to be removing powder from gloves. Powder binds to latex proteins and aerosolizes them when gloves are put on or removed. Powder-free gloves dramatically reduced airborne latex exposure, and sensitization rates in healthcare workers began falling within a few years.

By the mid-2000s, most major hospitals had transitioned to powder-free latex gloves (or to nitrile). The FDA formally banned powdered medical gloves in the US in 2016.

2. Low-Protein Latex Processing

Manufacturers developed new washing and leaching processes that reduced extractable protein levels in latex gloves. Modern low-protein latex gloves contain less than 50 µg/g of extractable protein (vs older gloves with 200-1000+ µg/g).

3. The Rise of Nitrile

Nitrile gloves, made from synthetic rubber, contain no latex proteins. They had been available since the 1990s but were initially expensive and lower quality. The latex allergy epidemic drove massive investment in nitrile manufacturing, and by the 2010s, nitrile gloves matched or exceeded latex in performance — at competitive prices.

Many hospitals that once used latex now use nitrile exclusively, eliminating latex allergy risk entirely from their facilities.

4. Awareness and Labeling

Latex-containing products are now required to be labeled. "Powder-free latex" is the default for medical gloves, and "latex-free" alternatives are widely available.

Where We Are Now

The epidemic has largely subsided in healthcare settings:

  • Healthcare worker sensitization rates have dropped back to 1-5% in most recent studies
  • Most major hospitals have transitioned to nitrile-only or low-protein powder-free latex
  • Powdered medical gloves are banned in the US, EU, and most other markets
  • Nitrile has become the default disposable glove in healthcare, food service, and many industrial settings

But latex allergy remains a significant concern for:

  • People with spina bifida and certain other congenital conditions (still high sensitization)
  • Workers in industries that still use powdered latex (some food processing, certain manufacturing)
  • People with repeated medical procedures (multiple surgeries, catheterization)
  • The general population — about 1-6% have some degree of latex sensitivity

Lessons Learned

The latex allergy epidemic taught us several important lessons:

  1. Occupational allergens are real and serious. When you massively increase exposure to a protein, sensitization rates will rise. The 1980s glove expansion was medically necessary but had an unintended consequence.
  2. Powder amplifies everything. Powder in gloves wasn't just a comfort feature — it was an exposure multiplier that drove sensitization rates dramatically.
  3. Synthetic alternatives can replace natural materials. Nitrile gloves are objectively better than latex for most applications today (no allergy, more durable, similar cost).
  4. Universal precautions saved lives but created new problems. The shift to universal glove use was correct, but the specific gloves chosen (powdered high-protein latex) caused harm that took a decade to address.

The Takeaway

Today, if you're setting up a healthcare facility, food service operation, or any workplace with regular glove use, the safe default is nitrile, powder-free. Latex gloves still have their place (some surgeons prefer them for tactile sensitivity), but they're no longer the default.
